Long-term acute care hospitals
HCA Virginia understands you may need long-term acute care when we release you from our hospital. Our HCA Virginia Plus Care Network helps you and your caregiver find the proper long-term acute care facility that best fits your health condition.
HCA Virginia Plus Care Network is pleased to offer this list of long-term acute care hospitals.
Long-term acute care hospitals provide treatment for patients with serious medical conditions who need ongoing care but who no longer require intensive care or extensive procedures. Patients often come from ICU and require more care than they can receive in a rehabilitation center.
HCA Virginia Plus Care Network shares this information to help you make an informed decision. It is the patient's right and responsibility to choose their post-hospital facility/agency.

Plus Care Network Facility/Agency Selection
Our hospital leadership teams evaluated both facility/agency-reported and publicly available data to select the TriStar Health Plus Care Network. More specifically, we selected facilities/agencies based upon their willingness to collaborate, quality scores, overall star rating, service offerings, and clinical capabilities. Overall, the hospitals chose providers that could meet the medical and psychosocial needs of their patients. TriStar Health Plus Care Network facilities/agencies have entered into an agreement with the hospital but do not pay to be a part of the TriStar Health Plus Care Network. Please note, some providers may be affiliated with, and/or owned or controlled, by the hospital. Facilities/agencies affiliated with, and/or owned or controlled by competitors may not be eligible for participation in the network.
